Request an Appointment

If your practice allows it, you may be able to request an appointment on MyHealthRecord.com.

To request an appointment:

1. Select My Appointments and then select Request.
2. Select the + New button.

3. On the next page, fill in the any required fields.

You may see different options available when you request an appointment. The available options may vary by provider.

4. After filling in any required information and any preferred information, select Submit.
5. You will then see a confirmation page.

Your practice may also allow you to request an appointment directly to your provider's schedule. If this option is available to you, follow the steps below.

1. Select My Appointments and then select Request.
2. Select the + New button.
3. Select a Reason for Visit (required). Your provider may also require you to select a provider.
4. Enter a preferred date.
5. Select Search.

6. On the next page, select a time slot from the provider's schedule (e.g., Wed Sep 12, 09:30 a.m.). Select the More button to view additional time slots (if available) for any day.

7. On the next page, review the scheduling information prior to submission. Enter in any additional information under Notes.

8. Select Submit when finished.
9. After submission, a confirmation page appears. Select Print to print the confirmation.
10. After you submit the request, your practice will contact you to schedule the appointment.
